We accept Medicare and most major PPO insurance plans for eligible Physical Therapy services, including Medicare • Anthem Blue Cross • Cigna • Health Net • Aetna • UnitedHealthcare • Hill Physicians • John Muir Physicians Network.
Insurance coverage varies by service and individual plan. If you plan to use insurance, please provide your current insurance information prior to your appointment so our team can verify benefits and determine whether a physician referral or insurance pre-authorization is required.
Please bring your photo ID and insurance card. Copays, deductibles, coinsurance, and non-covered services are due at the time of service.
We accept HSA/FSA payments and offer discounted cash-pay packages when appropriate.
